The Role of a Care Coordinator

In the Home Care Package team, Care Coordinators oversee and coordinate the delivery of care services to clients receiving home care. They assess clients' needs, develop care plans and coordinate with healthcare professionals to ensure the provision of tailored care. Care Coordinators also liaise with clients and their families to address concerns, monitor care delivery and make adjustments as needed to maintain quality of life and wellbeing in the comfort of their homes.

Experience in the Care Service Field

Tammy's nursing career began in 2001 in aged care, followed by roles at Nambour Hospital where she worked across medical, surgical, and transit units with QLD Health and the Ambulance Service. Transitioning to General Practice, Tammy found her calling in Home Care Packages and is now a valued member of the Burnie Brae Home Care team.

Beyond her dedication to aged care, Tammy enjoys exploring the ocean through swimming and snorkeling. She also shares her life with two pet snakes: Cecil and Baby, both beloved members of her family.
