Looking for extra support to stay active, social and doing the things you love? The Commonwealth Home Support Program (CHSP) offers tailored, subsidised assistance to meet your needs!

Whether you want to remain active in your community or require short-term help after an injury or illness, CHSP provides a variety of services designed to fit your lifestyle.

Allied Health

Achieve your health goals and improve your quality of life with our personalised treatment plans. Our skilled allied health professionals offer tailored care to help you maintain independence and confidence, whether you're recovering from an injury or looking to stay active in your community.

Social Support

Get involved at Burnie Brae or in your community through engaging events, activities, and social home visits that offer emotional support. Enjoy recreational support, outings with loved ones, and a variety of activities like arts and crafts, fitness, and day trips to exciting local destinations.

Community Transport

Stay connected in your community with our accessible transport options. Our friendly drivers ensure safe, comfortable travel to medical appointments, social events, shopping trips and more.

Note: Some services, like personal care, domestic assistance, and home maintenance, are currently at capacity.

Are you eligible for funding?

Eligibility for CHSP is determined by the Australian Government, and applications are made through My Aged Care. You must be aged 65 or older (50 or older for Aboriginal and Torres Strait Islander people), or younger if you are vulnerable.

CHSP is ideal for seniors needing one or two entry-level services. If you need more support, you can be assessed for a Home Care Package (HCP). If you’re currently waiting for your HCP allocation, you may also be able to access CHSP services in the meantime.

What are the costs involved?

While the Australian Government covers most CHSP costs, you may need to contribute if you can afford to. Your out-of-pocket expenses depend on the type and frequency of support, as well as your income. Before services begin, we’ll provide a clear cost breakdown.

If you need additional services beyond what is covered, you can also choose to pay privately.

How to apply for CHSP funding?

  1. Apply through My Aged Care
    Complete an online assessment or call 1800 200 422.
  2. Book Assessment
    If eligible, My Aged Care will arrange a home care assessment. Their assessor will contact you to arrange a suitable time to discuss your needs.
  3. Personalised Plan
    After the assessment, your assessor will create a tailored home support plan and provide a referral code to access services.
  4. Get Started
    Contact Burnie Brae
    with your referral code to discuss your needs and our available services.
