Eligibility for a Home Care Package

To be eligible for a Home Care Package, you must be an older person (aged 65 years and older, or 50 years and over for Aboriginal or Torres Strait Islanders) who requires coordinated services to live independently at home.

Check your eligibility online through the My Aged Care website to determine your eligibility for a Home Care Package.


Applying for a Home Care Package

Apply for an Assessment through My Aged Care

Start the process by applying for an assessment with the Aged Care Assessment Team (ACAT). You can do this online via the My Aged Care website or by calling 1800 200 422 (weekdays 8am - 8pm, Saturdays 10am - 2pm). If needed, a loved one can assist you with the application.

Complete an In-Home Assessment

After submitting your application, a local ACAT assessor will arrange a free in-home assessment. During this visit, they will evaluate your daily living needs and discuss the types of care you might qualify for. Discover tips on how to prepare and what to expect.


Approval & Package Allocation

Receive Your Home Care Package Approval

Once approved, you will receive a letter detailing your allocated level of care and your position on the national waiting list. You will also receive a referral code for your Home Care Package, which you will need to coordinate funding with your chosen care provider. Be aware that there may be a waiting period before your package and government funding are allocated.

Receive Your Allocation Letter

When your package becomes available, you will receive a confirmation letter with all the details. You will then have 56 days to select your care provider. If you choose Burnie Brae, contact us as soon as you receive your letter to start your services.


Starting Your Care Services

Create a Care Plan and Begin Your Services

Our dedicated Home Care Package team will arrange a meeting with you and your loved ones to discuss your priorities and complete the necessary paperwork. We will match you with suitable support staff to help achieve your goals and maintain regular visits to adjust your Care Plan and Home Care Individual Budget as needed.
